Each new patient must fill out the New Patient Registration forms prior to their first appointment. You may download or print the form then fill it in and bring it with you. Select “New Patient Forms”, listed in the right column on this page or ask us to mail the forms directly to you when you schedule your first appointment. If you are bringing in pre-filled forms to your appointment, please arrive 15 minutes early so that we may process your information and ask any additional questions that may arise, otherwise please allow 30 minutes to fill out the forms in the waiting room if you choose not to pre-register.

- Shoreline: Monday through Friday 7am to 5pm with extended hours available. (Note: This is where all surgeries are completed.)
- Woodinville: Monday, Wednesday and Friday 8am to 5pm.
Same day appointments are available. Walk-in appointments are accepted. An appointment is recommended whenever possible. (See Locations.)

If you are unable to keep a scheduled appointment, please notify us 24 hours in advance so that other patients can take your appointment time. If you call after hours, leave a voicemail for our staff or for an urgent issue requiring a physician, transfer to our answering service.
Since we do treat urgent conditions, there are times that our physicians may have to do emergency surgery. We do our best to minimize when this happens, but there may occasionally be longer waits if a physician is delayed in surgery or we may need to reschedule.
In the event that you have an urgent problem, one of our physicians will always be available to help.
