• Please bring the following to your appointment:

  • Any of the registration forms you are asked to bring in
  • A driver’s license or other valid picture ID
  • All of your health insurance cards
  • Physician referral (if applicable)
  • Any prior x-rays, MRIs, or other studies. These can be picked up from their source, usually with 2-4 days notice
  • Any prior operative reports for the condition
  • Worker’s compensation claim information, if applicable, including claim number, insurance company name and address, claim manager name and phone number.
